Sawyer Valentini is a bank worker who has moved from Boston to Pennsylvania to escape a stalker named David. She visits a support center, signs papers that she does not fully understand, and ends up locked inside a mental-health hospital. Sawyer sees David working there as an orderly, but the staff treat her warning as proof that she is unstable. A fellow patient helps her learn that the hospital keeps people for insurance money, and the film becomes a tense, claustrophobic fight for survival.
Unsane puts a woman’s fear of male control at the center of its story. David acts as if Sawyer belongs to him, while the hospital staff and authorities fail to take her danger seriously. The film repeats the idea that women can be ignored, labeled unstable, or punished when they ask for help. It also attacks a private health system that values insurance payments over care, but it presents these ideas through a violent thriller rather than speeches about party politics or identity groups.

Breakdown
These are the editorial factors and ratings behind our score for Unsane.
Woke representation / casting
Not present. The film has a mixed cast, including a black male patient and a female doctor, but it does not make race, disability, or diversity a story message.
0%
Woke political dialogue
Sawyer asks where the outrage and advocate are after she says she was sexually assaulted. She uses the moment to frame her treatment as a public failure to listen to women. Other talks about stalking, drugs, and insurance explain the plot instead of giving political speeches. This leaves one clear activist-style speech inside a much larger thriller.
24%
Identity-driven story themes
Sawyer’s conflict centers on a woman who is stalked and then treated as unreliable when she asks for help. David treats her as something he owns, so the film links the danger to male entitlement. The staff and authorities keep asking Sawyer to prove herself instead of dealing with the threat. Sawyer finally takes control of the fight, giving the story a clear woman-survives-abuse message.
58%
Western institutional / cultural critique
Highland Creek keeps patients confined so it can collect private insurance payments. The staff use forms, medicine, and locked rooms to control people instead of helping them. David also turns a workplace and hospital into tools for male control over Sawyer. The film makes these failures feel like part of a wider problem with health care and the way women are heard.

Woke character or canon changes
Not relevant. The movie is an original story. It does not change a known character, book, or real person.
0%
Anti-woke backlash and complaints
Not present. The public material reviewed mostly debates plot logic, acting, the iPhone look, and the ending. No notable anti-woke or right-leaning complaint about DEI, identity politics, or the film’s gender themes was found.
